Advanced Health Care Directive
At age 18, youth reach a very important milestone in their lives. Becoming an adult brings new opportunities as well as new responsibilities and consequences. For most purposes, at age 18 individuals are treated as adults. The right to decide on your own health care is just one of the very important rights that transfer to an adult at the age of majority.
In cases where a families input may be needed specific to health care, an advanced Health Care Directive Form (SFL-461) must be completed and on file with CA Secretary of State, Special Filings Unit, P.O. Box 942877, Sacramento, CA 94277-0001.
By completing this form you name another individual as agent to make health care decisions for you if you become incapable of making your own decisions or if you want someone else to make those decisions for you now even though you are still capable.
